These items are Included in this plan but not covered by insurance:

• Provider available for you by text, phone, email 24/7

• Guaranteed unlimited same or next day Video appointments

• Unlimited contact with provider

• All FocusFit Features

• Medical paperwork such as FMLA, insurance authorizations, disability, etc.

• Resources and advice to save you money on your medical needs (lab work at wholesale prices, etc.)

• We will make every effort to minimize out of pocket costs.

These items are also included but we are required to bill insurance:

• Primary Care services:

  • Chronic Disease Management: Expert guidance for conditions such as Diabetes, High blood pressure, High cholesterol, mental health.

  • Acute Illness Telehealth Using the Urgent Care Video Visit: UTIs, Colds, Ear infections, Strep throat, sinus infections, or other sudden issues can be handled virtually.

  • Acute pain control

  • Electronic prescriptions: prescriptions for medications, blood work, labs, advanced studies

  • Referrals and Consultation with the specialist by your provider

• In-person (clinic or home visits)

  • Annual physicals

  • Injections available (at cost)

For patients without insurance:

You get all the same services that’s included in the plans however the cost of care is often higher because we don't have the support of insurance companies to help cover a portion of the fees. We always strive to offer the best possible care at a fair price, and we're here to work with you to ensure your needs are met.
